Consult with the Office for Harassment Prevention and Counseling

Office hours are subject to change.

①Office Hours: Monday - Friday 10:00 am - 5:00 pm (except for holidays)
Appointments can be made for the following timings:
Ito Campus: 10:00 am - 5:00 pm
Hospital Campus: 10:30 am - 4:00 pm
Online: 10:00 am - 5:00 pm

Consultations are by appointment only.
If you would like to book a consultation, please make an appointment by phone or email in advance.
※The consultation will be conducted in Japanese.If you have any concerns about the consultation in Japanese, we will ask that you bring an interpreter with you.
※We will ask you to submit a consent form and other documents in advance of the online consultation. If you would like to conduct an online consultation, please refer to the Cautions on Conducting Online Consultations before making an appointment.

②We accept consultation appointment application forms (these can only be sent within the university system), and requests by telephone, fax, and email. Information in the consultation appointment application forms is encrypted, so we recommend using them.
If you use the consultation appointment application form, please fill in the required fields before sending.
When applying through telephone, fax, or email, please mention your name, contact information (cell phone number or email address, etc.), the date and the campus where you would like your consultation to be held, and a brief description of the matter that you would like to discuss.

③Infectious Disease Control
Our office is equipped with disinfection, ventilation, and other infection control measures.
If you have a fever or cold symptoms, please refrain from coming to the office and reschedule your visit.

④Handling of personal information when setting up a consultation appointment
This office recognizes the importance of protecting the personal information of people who come for consultations. Personal information will be securely protected on our website in accordance with the “Kyushu University Personal Information Management Regulations.”

※Regarding use of SSL (encrypted communication)
To securely exchange data and protect the personal information of people requesting consultations, we use encrypted communication via SSL (Secure Socket Layer) for the consultation appointment application form, and we protect the information entered in the application forms so that it can be sent securely to our web server.

Requests for consultation appointments using the consultation appointment application form can only be made from within the university.
